Maxillary vein ทอดร่วมกับ 1st part of maxillary artery อยู่ระหว่าง sphenomandibular ligament และ neck of mandible หลังต่อ neck of mandible, maxillary vein รวมกับ superficial temporal vein form เป็น retromandibular vein แขนงต่างๆ ของ maxillary vein รวมกัน form เป็น pterygoid venous plexus

Pterygoid venous plexus อยู่รอบ lateral pterygoid muscle ประกอบด้วย veins ต่างๆ ซึ่งทอดร่วมกับแขนงของ maxillary artery drain สู่ maxillary vein ทางด้านหลัง

Communications of the pterygoid venous plexus Cavernous sinus ทาง emissary vein Inferior ophthalmic vein ทาง vein ที่ผ่านมาทาง inferior orbital fissure facial vein ทาง deep facial vein

Maxillary artery (Internal maxillary artery) เป็น terminal branch of the external carotid artery แยกจาก external carotid artery หลัง neck of mandible ผ่านเข้าสู่ infratemporal fossa ระหว่าง neck of mandible และ sphenomandibular ligament

Parts of the maxillary artery Divides into 3 parts by lower head of lateral pterygoid muscle 1st part (mandibular part) - lateral to lateral pterygoid muscle 2nd part (pterygoid part) - superficial or deep to the muscle 3rd part (pterygopalatine part) - medial to the muscle

Branches of the maxillary artery A. Branches from the 1st part 1. Deep auricular artery 2. Anterior tympanic artery 3. Middle meningeal artery 4. Accessory meningeal artery 5. Inferior alveolar (dental) artery

Middle meningeal artery Passes upwards between 2 roots of auriculotemporal nerve passes through foramen spinosum supplies dura mater 2 cm. above the zygomatic arch, divides into frontal & parietal branches

Frontal (anterior) branch of middle meningeal artery lies on the internal aspect of the pterion and passes to the midpoint of inion and nasion

Parietal (posterior) branch passes backwards on squamous part of the temporal bone to lambda

B. Branches from the 2nd part 1. Deep temporal branches 2. Pterygoid branches 3. Masseteric branches 4. Buccal artery

Mandibular nerve Nerve of the mandibular arch Consists of a large sensory component and a smaller motor component

Sensory fibers of mandibular nerve receive sensory sensation from - skin & mucous membrane of the cheek & lower lips - skin on the chin - external acoustic meatus - lateral aspect of the tympanic membrane - parotid gland

Sensory fibers of mandibular nerve - TMJ - lateral aspect of the scalp - all mandibular teeth & gingivae - anterior 2/3 of tongue - floor of the mouth

Motor fibers of mandibular nerve Supply muscles of mastication tensor tympani muscle tensor veli palatini muscle mylohyoid muscle anterior belly of digastric muscle

Origin & course Sensory root arises from trigeminal ganglion Motor root joins sensory root, passes through foramen ovale and form nerve trunk or stem of the mandibular nerve

Enters infratemporal fossa deep to lateral pterygoid muscle Mandibular nerve devides into an anterior and a posterior divisions

Branches of the mandibular nerve From the nerve trunk 1. meningeal branch (nervus spinosus) 2. nerve to medial pterygoid

From the anterior division 1. Nerve to masseter 2. Deep temporal nerves (anterior & posterior) 3. Nerve to lateral pterygoid 4. Long buccal or buccal or buccinator nerve

From the posterior division 1. Auriculotemporal nerve 2. Lingual nerve 3. Inferior alveolar nerve - mylohyoid nerve - mental nerve - incisive nerve

Otic ganglion เป็น parasympathetic ganglion อยู่ใน infratemporal fossa ยึดติดกับ medial surface ของ mandibular nerve ใต้ foramen ovale พอดี

Nerve fibers ที่ผ่านเข้าสู่ otic ganglion 1. Parasympathetic fibers Preganglionic parasympathetic fibers ผ่านมาทาง tympanic branch of glossopharyngeal nerve tympanic plexus (ใน middle ear cavity) lesser petrosal nerve synapse กับ cells ใน otic ganglion

Postganglionic parasympathetic fibers auriculotemporal nerve parotid gland

2. Sympathetic fibers จาก plexus บน middle meningeal artery (postganglionic sympathetic fibers จาก superior cervical ganglion) 3. Motor fibers จาก nerve to medial pterygoid muscle ไปเลี้ยง กล้ามเนื้อ tensor tympani และ tensor veli palatini 4. Sensory fibers of Cr.V และ Cr.IX

Nerve fibers ในข้อ 2, 3 และ 4 ผ่าน otic ganglion โดยไม่ได้ synapse กับ cells ใน ganglion

2.5 dissect superficial contents in the infratemporal fossa
