Quality of life and coordination of care for nursing home residents reflects their person-centered goals of care, care needs, and preferences and wishes. Promote nursing homes as a well-developed aspect of the continuum of care, including end of life care. Increase exchange of information amongst providers as patients transition between care settings. Increase opportunity for residents and families to drive individual care planning. Provision of information to the resident and potential caretakers in a way that they can understand and access. Ensure residents are in most appropriate care setting and are made aware of home and community based services. Goal 1 29
